This booklet does not contain the legal forms you will need if you wish to appoint someone to support you to make decisions. There are links to these forms at the bottom of this page.

Advance care planning and forms
The Health Vic website has links to forms for documenting your decisions about medical treatment in future if you are not able to communicate your wishes, or if you no longer have decision making capacity.
You can download the following forms:

- Appointment of medical treatment decision-maker form (there are short and long versions of this form).
